Inflammaging describes a chronic, systemic, low-grade inflammatory state recognized asa major risk factor for age-related diseases (ARDs). Since its identification 25 years ago, ahighly complex network of inflammatory mechanisms has emerged, shaped by the lifelongexposure to internal and external stimuli related to the individual-specific “immunobiographyā€. Thus, inflammaging is not only highly personalized but alsohistorically and geographically heterogeneous, reflecting the unique combination ofgenetics, lifestyle and environmental exposure. Recently, we applied XAI (eXplainableArtificial Intelligence) to develop an inflammaging clock able to assess the inflammatoryage of a single person, complementing a previous inflammaging clock highly correlatedwith ARDs. Merging these clocks into a new one, informative, cost-effective, and possiblyorgan-specific-refined, is a priority, to integrate inflammaging into clinical geroscienceresearch, by routinely quantify/monitor in single persons the efficacy of anti-inflammaginginterventions (diets and gut microbiome, physical exercise, geroprotectors, senolytics andmonoclonal inhibitors of pro-inflammatory cytokines).
